【文章內(nèi)容簡介】 Ethotoin 對(duì)大發(fā)作和復(fù)雜部分性發(fā)作有效。由于作用弱，只作輔助藥物與其他藥合用 不良反應(yīng)少見 (1) (2) 藥物 應(yīng)用 不良反應(yīng) Ethosuximide 對(duì)小發(fā)作不及氯硝西泮(clonazepam)，但副作用及耐受性較少，故仍為防治小發(fā)作的首選藥 常見為胃腸道反應(yīng)，其次為中樞神經(jīng)系統(tǒng)癥狀如頭痛，偶見粒細(xì)胞缺乏癥和再障 Benzodiazepine diazepam首選藥， nitrazepam主要用于小發(fā)作和非典型失神性發(fā)作， clonazepam和 clobazam diazepam靜過快可致呼吸抑制，故應(yīng)緩慢靜脈注射。其他見鎮(zhèn)靜催眠藥 Valproate 作不及 phenytoin和phenobarbital；對(duì)小發(fā)作療效優(yōu)于 ethosuximide 常見有惡心 、 嘔吐等，CNS反應(yīng)少，約 25%的患者有肝毒性 Carbamazepine 精神運(yùn)動(dòng)性發(fā)作和大發(fā)作療效較好 眩暈、惡心、嘔吐，少數(shù)可有粒細(xì)胞和血小板減少。 癲癇類型 抗癲癇藥 典型大發(fā)作 苯妥英鈉 、卡馬西平（酰胺咪嗪） 丙戊酸鈉 小發(fā)作 乙琥胺 、丙戊酸鈉、苯二氮卓類 精神運(yùn)動(dòng)性發(fā)作 卡馬西平 、苯妥英鈉、苯巴比妥 丙戊酸鈉 部分性發(fā)作 苯妥英鈉 、撲米酮 癲病持續(xù)狀態(tài) 地西泮 、異戊巴比妥、 苯巴比妥、苯妥英鈉 1. 合理選擇藥物和劑量 選用一種有效藥物，先從小劑量開始，逐量增加至獲得理想療效而不產(chǎn)生嚴(yán)重不良反應(yīng)的有效劑量。若一種藥難以奏效或治療混合型患者，常需聯(lián)合用藥。 2. 合理療程 在治療過程中不宜隨便更換藥物，必要時(shí)采用過渡用藥方法，即在原藥的基礎(chǔ)上加用新藥，待新藥發(fā)揮療效后再逐漸停用原藥。癥狀完全控制后，還要維持治療 2～ 3年再逐漸停藥，以防復(fù)發(fā)。 3. 注意不良反應(yīng) 終生用藥。 Table 162 (1) Summary of Charateristics of Some Newly Developed Antiepileptic Drugs drugs cellular mechanisms main uses mainly unwanted effects antiepilepsirine increase in content of 5serotonin。 broad therapeutic profile。 relatively free of sideeffects, drowsiness etc. vigabatrin inhibition of GABA transaminase。 effective in patients unresponsive to conventional drugs。 drowsiness, behaviour and mood changes lamotrigine inhibition of Na+ channels。 broad therapeutic profile。 hypersensitivity reactions (especially skin rashes) Table 162 (2) Summary of Charateristics of Some Newly Developed Antiepileptic Drugs drugs cellular mechanisms main uses mainly unwanted effects felbamate unknown。 broad therapeutic profile。 use limited to intractable disease, owing to risk of hypersensitivity reactions resulting in aplastic anemia gabapentin unknown。 partial seizures。 safe in overdose. relatively free of sideeffects Table 162 (3) Summary of Charateristics of Some Newly Developed Antiepileptic Drugs drugs cellular mechanisms main uses mainly unwanted effects tiagabine GABA uptake inhibitor。 partial sei
