For the patient in the vigte, I would begin with 25 mg of promethazine intramuscularly, which is usually effective for suppressing vertigo and vomiting. If the vomiting persists, I would add 10 mg of prochlorperazine intramuscularly. Once the vomiting subsides, less sedating oral antivertiginous drugs such as 25 mg of meclizine or 50 mg of dimenhydrinate can be used, but only for a few days, until the acute vertigo subsides. Although corticosteroids and antiviral agents have been suggested for treatment of vestibular neuritis, so far there have been no definitive studies demonstrating the efficacy of these medications.
