【正文】 ,間接熒光試驗(yàn)大于 1:16 病 理 pathology ?急性間質(zhì)性肺炎及支氣管炎 ?12周吸收 ?病變自限性 影像學(xué)表現(xiàn) Imaging manifestations ?早期病變 : 肺紋理增粗、模糊 Intensification of lung markings 網(wǎng)狀紋理 fine streaklike, or nodreticular shadows ?病變發(fā)展后 :Small or large patchy opaque ?胸腔積液 ,肺門縱隔淋巴結(jié)腫大少見 . 肺 膿 腫 (lung abscess) ? 由化膿性細(xì)菌引起 ,早期為化膿性肺炎 ,壞死液化 ,形成膿腫 Purulence bacteriainflammation focusNecrosis and fluidifyabscess ?infect approach： ? inhale ? hematogenous ? direct spreed Stage： acute and chronic stage lung abscess 臨 床 ? 急性期 Acute stage： 有急性化膿性肺炎的表現(xiàn)，高熱寒戰(zhàn)、咳嗽咳痰、胸痛、 咳大量膿臭痰 白細(xì)胞及中性粒細(xì)胞明顯增高 ? 慢性期 Chronic stage： After 3 months the abscess was considered to be chronic 經(jīng)?？人浴⒖饶撎岛脱?， 不規(guī)則發(fā)熱及貧血消瘦等 lung abscess 病 理 ? 化膿性肺炎（ 滲出和實(shí)變 ） —細(xì)支氣管阻塞、小血管炎性栓塞 —壞死 液化 —形成空洞 (膿腔 ) ? 膿液破潰到胸腔 —膿氣胸、支氣管胸膜瘺 ? 持續(xù)不愈超過 3個(gè)月 轉(zhuǎn)為慢性，膿腔 周圍炎性浸潤減少 但纖維組織增生 lung abscess 急性期 Acute stage排膿之前 ： 化膿性肺炎 團(tuán)狀致密影 ,邊緣模糊 X 線 表 現(xiàn) 急性期 排膿以后 邊界模糊的團(tuán)狀陰影內(nèi)有透亮的空洞影，空洞壁較厚，內(nèi)壁較光整，常伴有液平面 Cavity thick wall with airfluid level surrounded by exudative lesion lung abscess CT features airfluid level 肺膿腫 CT表現(xiàn) 較早發(fā)現(xiàn)病變中央的較低密度液化壞死區(qū) thick irregular wall a low density center(central necrosis) or airfluid level peripheral enhancement Lung abscess in the right lower lobe. A thick irregular wall cavity a lowdensity center (central necrosis) containing air fluid level. peripheral enhancement adjacent pulmonary consolidation and atelectasis small pleural effusion 慢 性 期 Chronic stage ?膿腔 周圍炎性浸潤減少 —邊緣較急性期要清晰 ?空洞周圍纖維組織增生形成厚壁空洞 ?膿腔 周圍 纖維組織增生 —有較多紊亂的條索狀影 fibrotic lesions ?竇道 — 形成多房性膿腫 ?牽引性支氣管擴(kuò)張 ?并發(fā)膿胸與胸膜增厚 Thickened pleura lung abscess 空洞周圍纖維組織增生形成厚壁空洞 one or more irregular cavities and with multiloculation some fibrotic lesions by the cavity or in the cavitary wall Thickened pleura are usually seen 血源性肺膿瘍： Hematogenous lung abscess 膈下膿腫或肝膿腫擴(kuò)展到肺形成肺膿腫：multiple abscesses in both lungs 肺膿瘍 空洞 壁厚，內(nèi)壁較光滑整齊，外緣模糊 明顯液平面 結(jié)核性 空洞 壁薄，內(nèi)外壁較光整，常有衛(wèi)星灶 無或僅少許液體 癌性 空洞 壁厚，內(nèi)外壁凹凸不平，有壁結(jié)節(jié)， 外壁可見分葉毛刺 空洞 鑒 別 診 斷 lung abscess 等續(xù)
