If a PTW is not applied as required, a fine of 200RMB shall be imposed on the person held responsible. Starting work before the work permit is issued/revalidated will be deemed the same as above. A fine of 200RMB shall be imposed on the task supervisor.第4條： 簽名視為唯一的授權(quán)確認(rèn)，未經(jīng)書面允許代替他人簽名的，對責(zé)任人員罰款100元。Signature is regarded as the only authorization of PTW. A fine of 100RMB shall be imposed on person held responsible for signing PTW for others without official delegation.第5條： 在拿到許可證之后工作之前須由作業(yè)主管召集所有作業(yè)人員召開工具箱會(huì)，沒有召開的，發(fā)現(xiàn)一次對作業(yè)主管罰款200元。Once the work permit is issued, prior to mencing the job, Task supervisor shall have a toolbox meeting with all people involved. If the toolbox meeting is not held, task supervisor shall be penalized with a fine of 200RMB第6條： 工作許可證應(yīng)定期由HSE專員組織生產(chǎn)和維修現(xiàn)場管理人員進(jìn)行抽樣審核，未完成月度審核目標(biāo)，對HSE負(fù)責(zé)人罰款200元，無故拒不參加審核者罰款200元 。HSE personnel shall facilitate the regular PTW audits according to a agreed schedule and the production/maintenance site management team shall be involved. if the schedule not followed, HSE manager shall impose a fine of 200RMB. The designated person who refuse to present himself shall be penalized with a fine of 200.第7條： 屬地負(fù)責(zé)人應(yīng)確認(rèn)所有經(jīng)手的許可證不存在沖突作業(yè)，否則罰款200元。Area responsible shall ensure no conflicts/interference between works and other concurrent operations. A 200RMB fine shall be imposed if failed to indentify the likely risks. 第8條： 作業(yè)主管應(yīng)檢查并確認(rèn)該作業(yè)所用的所有工具與設(shè)備處于完好狀態(tài)，特別是對承包商的設(shè)備和工具。發(fā)現(xiàn)有缺陷的工具投入使用的，對作業(yè)主管罰款200元。Task supervisor shall check and ensure the tools and equipments used for the tasks are in good working conditions, especially for the contractor’s tools and equipments. If any unsafe tools and equipments are found in use, the task supervisor shall be penalized with a fine of 200RMB.第9條： 承包商工頭應(yīng)確保本組所有人員都已經(jīng)經(jīng)過哈納斯的入場培訓(xùn)，否則禁止申請任何工作許可。如有違反，對承包商工頭罰款200元。Contractor foreman shall ensure everyone in his group pleted the Hanas LNG plant HSE induction. Contractor foreman shall be imposed a fine of 200RMB for the violation. 第10條： 高風(fēng)險(xiǎn)作業(yè)的作業(yè)許可可能會(huì)要求安全監(jiān)護(hù)人在場。沒有按照作業(yè)程序指定安全監(jiān)護(hù)人的，對作業(yè)主管罰款200元，指定了安全監(jiān)護(hù)人，而沒有履行安全監(jiān)護(hù)人職責(zé)的，對指定的安全監(jiān)護(hù)人罰款200元。High risk jobs may require safety watcher/guard. A fine of 200RMB shall be imposed if task supervisor fail to nominate the safety watcher in accordance with HSE MS. A fine of 200RMB shall be imposed if the nominated safety watcher/guard fail to perform his duty.第11條： 一些工作許可會(huì)要求測氣，沒有根據(jù)許可要求的檢測氣體，或者檢測頻率達(dá)不到要求的，對責(zé)任人罰款200元。Some jobs may require gas testing. Fail to perform gas testing or the frequency of gas testing can’t meet the PTW requirement, responsible party shall be imposed a fine of 200RMB第12條： 屬地負(fù)責(zé)人和作業(yè)主管及承包商工頭應(yīng)檢查并確認(rèn)現(xiàn)場工作準(zhǔn)備完全符合工作許可證關(guān)于安全作業(yè)的要求。如果工作開始后現(xiàn)場安全措施發(fā)現(xiàn)低于許可證的要求。對作業(yè)主管和承包商工頭每人罰款200元，對屬地負(fù)責(zé)人罰款100元。Area responsible, task supervisor and contractor foreman shall check and ensure that all specified requirements laid down in the PTW are implemented before starting the work. In case control measures specified in PTW are found not in place after starting the job, task supervisor and contractor foreman shall be imposed a fine of 200RMB, and area responsible shall be imposed a fine of 100RMB.第13條： 所有的工作許可在工作結(jié)束后應(yīng)立即關(guān)閉，未及時(shí)關(guān)閉者，對作業(yè)主管罰款200元。未遵守延時(shí)程序，暫?；蛘咧匦律С绦虻?，對作業(yè)主管罰款100元。All permits shall be closed once the job is done. Fail to close the permits in time, task supervisor shall be penalized with a fine of 200RMB. For failure to follow the PTW extension, suspend and revalidation procedure, task supervisor shall be imposed a fine of 100RMB第14條： 已經(jīng)申請工作許可，但未在工作現(xiàn)場張貼的。對作業(yè)主管罰款50元。PTW is issued but not posted on site, task supervisor shall be imposed a fine of 50RMB 第15條： 作業(yè)主管，HSE經(jīng)理和屬地負(fù)責(zé)人必須參加日常協(xié)調(diào)會(huì)議，無故不參加者或者因有事無法參加而且沒有指定代理人的，罰款50元。Task supervisor, HSE manager and area responsible shall attend the daily PTW coordinating meeting. A fine of 50RMB shall be imposed for absence without excuse or absence without delegation.第16條： 所有發(fā)出的工作許可應(yīng)記錄在工作許可記錄簿中，發(fā)現(xiàn)記錄不全或者無記錄的，對當(dāng)值HSE專員罰款50元。All permits issued shall be recorded in the log book.
