Verification證件核實(shí)a) Prior to employment, engagement or work on board, seafarer should present his or her identification document to Marine Personnel Department，issued by Competent Authority, confirming his or her age， the following document include but not limit to:受雇，受聘或到船工作前，海員應(yīng)向船員部出示由主管當(dāng)局簽發(fā)的證明文件，以證實(shí)自己的年齡。這些文件包括但不限于:i. Seaman’s Passport 海員證ii. Seafarer’s Identity Document 服務(wù)簿b) Marine Personnel Department check the certificate to determine whether the basic information, such as Date of birth, Name, etc. are consistent with other document produced by seafarer, where certificate forged or altered, the seafarer should not be employed. 船員部對(duì)上船海員持有的證書進(jìn)行核對(duì)，核對(duì)其與其他證書的基本信息是否一致，這些信息包括海員出生日期，姓名等。如有偽造,涂改的情況，不予錄用。c) Master should check seafarer signon against valid identification documents to ascertain if the holder of certificate is him/her。 When the documents not being in conformity with the holder physical attributes， or forged document， master should inform shipowner as soon as possible so as to take appropriate measures by shipowner船長(zhǎng)對(duì)到船工作海員的證件進(jìn)行核對(duì),確認(rèn)證件持有者是否本人，如果發(fā)現(xiàn)與本人體貌特征情況不符或偽造證件時(shí)，立即報(bào)告公司以便公司采取相應(yīng)的措施3. Document Keeping文件記錄a) Marine Personnel Department keep the Scanning softcopy of abovementioned Certificate of Seafarer。船員部保留海員上述證件電子版掃描件。b) Marine Personnel Department keeps the sign on/sign off record when seafarer changes.安排海員上船下船時(shí)，船員部保留受雇，受聘或到船工作海員的上下船動(dòng)態(tài)記錄c) Master update the crew list immediate after crew change，date of birth of each seafarer is noted against his/her name on the crew list.海員更換后，船長(zhǎng)立即對(duì)海員名單進(jìn)行更新，每一海員的出生日期應(yīng)在海員名單中的姓名旁標(biāo)注. d) Master or officer acting on his or her behalf (“petent officer”) verify the crew list at proper interval and record the date of such verification.船長(zhǎng)或代表船長(zhǎng)的高級(jí)海員（“主管高級(jí)海員”）定期核查海員名單，并記錄核查的日期e) Master should keep employment records of seafarer, include the signoff seafarer, these records include but not limit to the following:船長(zhǎng)應(yīng)保持海員受雇在船的記錄，包括已經(jīng)下船海員的受雇記錄，這些記錄包括但不限于：i. Prior to employment on board Seafarers should be in possession of a valid medical certificate and Vaccination/ Prophylaxis Certificate attesting that they are medically fit to perform the duties they are to carry out at sea.海員上船工作前應(yīng)持有有效的體檢證書和預(yù)防接種證書，證明其健康狀況適合其將在海上履行的職責(zé)。2. Verification 證書核查a) Marine Personnel Department should focus on the following：船員部應(yīng)重點(diǎn)關(guān)注：i. Marine Personnel Department check the medical certificate and Vaccination/ Prophylaxis Certificate to determine whether the basic information, such as Date of birth, Name, etc. are consistent with other document produced by seafarer, where certificate forged, altered, the seafarer should not be employed. 船員部對(duì)上船海員持有的體檢證書和預(yù)防接種證書進(jìn)行核對(duì)，核對(duì)其與其他證書的基本信息是否一致，這些信息包括海員出生日期，姓名等。如有偽造,涂改的情況，不予錄用。ii. General ment on the medical certificate generated by medical practitioner to determine whether a potential employee is fit for duty.體檢證書上體檢醫(yī)生給出的結(jié)論，確定未來(lái)雇員的健康狀況是否足以勝任其工作iii. Validity data證書的有效期iv. A English version must be provided / attached必須提供英文版本。v. The medical certificate shall ply with the Requirements issued by the Competency Authority specifying the standards of medical fitness for seafarer in accordance with the STCW Convention 1978, as amended from time to time. 體檢證書要符合主管當(dāng)局根據(jù)修訂的1978 年STCW 公約制定的對(duì)海員健康標(biāo)準(zhǔn)的規(guī)定vi. The medical certificate shall be issued by a duly qualified medical practitioner or, in the case of a certificate solely concerning eyesight, by a person recognized by the petent authority as qualified to issue such a certificate.體檢證書應(yīng)由有正規(guī)資格的醫(yī)師簽發(fā)，或者，對(duì)于只涉及視力的證書而言，由經(jīng)主管當(dāng)局認(rèn)可的具備簽發(fā)證書資格的人員簽發(fā)。b) Master should check seafarer signon against medical certificate and Vaccination/ Prophylaxis Certificate to ascertain if the holder of certificate is fit for his/her duty。When the documents forged or altered, master should inform shipowner as soon as possible so as to take appropriate measures by shipowner.船長(zhǎng)對(duì)到船工作海員的體檢證書和預(yù)防接種證書進(jìn)行核對(duì),確認(rèn)證件持有者適任，如果發(fā)現(xiàn)證書涂改或者偽造時(shí)，立即報(bào)告
