【正文】 information upon request of Insurers其他保險人需要的資料 Note 重要通知a)The Insured should not disturb the incident scene, 被保險人不得破壞現(xiàn)場 b)It is the duty of the Insured to minimize the extent of loss and safeguard all damaged ，保護(hù)受損財產(chǎn)是被保險人的義務(wù)c)Do not throw away any damaged property until the Insurers has inspected the damaged item and consented to remove it from your ，被保險人不得擅自放棄受損財物d)To take photos of the damaged items immediately after the loss/damage is （財物）受損，應(yīng)將受損物品拍照。其他人是否對受損財產(chǎn)有利益存在e)Whether there is any other insurance in to the Insurers a detailed claim in writing containing:以書面形式向保險人提供詳細(xì)之索賠清單，包括：a)List of property destroyed or damaged。房屋的占用情況及性質(zhì)c)Capacity in which the insured claims(whether for example, as owner, or mortgagee)。 claim form is filled and this will provide the insurers with the following information:向保險人提供包括下列內(nèi)容的表格：a)Date, cause, and circumstances of the incident。第二篇：財產(chǎn)保險索賠程序財產(chǎn)保險索賠程序（中英文雙語版）財產(chǎn)保險索賠程序（中英文雙語版） a loss is know to have occurred which may give rise to a claim under the Policy, you should notify Brokers, Agents, Insurers and report to Police Station(in case of burglary, theft, fire, malicious damage to other similar nature)immediately by phone and give contact phone number and ，應(yīng)當(dāng)立即通知保險經(jīng)紀(jì)人、代理人、被保險人；如案件涉及盜竊、火災(zāi)，或惡意破壞時應(yīng)報告公安機(jī)關(guān)并留下聯(lián)系電話和聯(lián)系人。三、學(xué)平險因意外或疾病死亡保險理賠提供材料如下：保險單，申請書，學(xué)校證明，申請人身份證復(fù)印件，被保險人戶籍注銷證明復(fù)印件，銀行卡復(fù)印件，居民死亡證明原件，火化證明原件，土葬請本村委開證明并加蓋公章，出險人 父母雙方身份證復(fù)印件。一、意外傷害醫(yī)療保險理賠提供材料如下：保險單復(fù)印件、學(xué)校證明（帶好學(xué)生證到學(xué)工處教育管理科開具，并在證明上留下聯(lián)系電話）、申請人身份證復(fù)印件、被保險人身份證（或戶口）復(fù)印件，學(xué)生證復(fù)印件、銀行卡復(fù)印件（賬號復(fù)印不清楚請在賬號下方用筆填寫清楚），疾病證明書，病歷，檢查報告，醫(yī)療費(fèi)發(fā)票等。第一篇：索賠申請書及索賠須知中國人民財產(chǎn)保險股份有限公司索賠申請書及索賠須知中國人民財產(chǎn)保險股份有限公司投保人、被保險人或者受益人應(yīng)于知道保險事故發(fā)生之日起兩天（48小時）之內(nèi)通知保險公司。理賠時請攜帶以下材料到保險公司，每周五下午，保險公司工作人員會在學(xué)生工作處學(xué)生事務(wù)中心辦公室為同學(xué)服務(wù)。二、住院醫(yī)療保險理賠提供材料如下：保險單復(fù)印件、學(xué)校證明（帶好學(xué)生證到學(xué)工處教育管理科開具，并在證明上留下聯(lián)系電話）、申請人身份證復(fù)印件、被保險人身份證（戶口）復(fù)印件、學(xué)生證復(fù)印件、銀行卡復(fù)印件（賬號復(fù)印不清楚請在賬號下方用筆填寫清楚）、疾病證明書、檢查報告、出院小結(jié)、費(fèi)用清單、醫(yī)療費(fèi)發(fā)票等。四、如有在城鎮(zhèn)居民醫(yī)療保險及農(nóng)村合作醫(yī)療保險報銷的：申請書，學(xué)校證明，申請人身份證復(fù)印件、被保險人身份證（戶口）復(fù)印件，（疾病證明，費(fèi)用清單，出院小結(jié)及醫(yī)療費(fèi)發(fā)票復(fù)印件并加蓋城鎮(zhèn)居民醫(yī)療保險或農(nóng)村合作醫(yī)療保險公章并注明復(fù)印件與原件一致），并提供報銷核單。 notice thereof in writing to the Insurers together with a Claim Form and shall within 30 days after such destruction or damage, or such future time as the Insurers may in writing ，并在保險公司允許的時間內(nèi)（一般為30天）填寫出險通知書和索賠清單。事故發(fā)生的時間、原因及損失程度b)Situation and occupancy of the premises。索賠人的地位，舉例說是業(yè)主或是抵押債權(quán)人d)Whether any other person is interested in the property damaged。損毀財物的清單b)The amount of destruction or damage thereto respectively having regard to their value at the time of the destruction or damage受損毀時的數(shù)量和價值 properties/scene affected and make them available for inspection by a representative or surveyor of the Insurers for a reasonable period of time but the Insured shall not in any case be entitled to abandon any property to the Insurers whether
