II 、 Ⅲ 導(dǎo)聯(lián)呈 QR型或出現(xiàn)高大的 R波。 （ 3）預(yù)激綜合征 Preexcitation syndrome, Wolff ParkinsonWhite syndrome, WPW syndrome 在正常的房室傳導(dǎo)通路之外，激動通過旁路傳導(dǎo)束提前到達(dá)，使部分（或全部）心室肌預(yù)先激動，形成預(yù)激綜合征。 旁路傳導(dǎo)束 bypass tract 預(yù)激綜合征 Preexcitation syndrome, Wolff ParkinsonWhite syndrome, WPW syndrome 心電圖特征 1. 在 QRS波之前出現(xiàn)“ Δ”（ delta）波； 2. P－ R間期縮短 ()，但 P－ j間期正常； 3. QRS波增寬； 4. 常有繼發(fā)性 ST－ T波改變。 “Δ”（ delta）波 J PJ 正常 預(yù)激綜合征 Preexcitation syndrome, Wolff ParkinsonWhite syndrome, WPW syndrome 根據(jù)預(yù)激向量方向的不同，一般分為 A、B、 C三型。 預(yù)激綜合征 Preexcitation syndrome, Wolff ParkinsonWhite syndrome, WPW syndrome A型：“ Δ向量對向左前，使胸前導(dǎo)聯(lián)心電圖均呈 R型，常以 V3R為最高?！?Δ波均向上。 V1 V2 V3 V4 V5 V6 Ⅰ Ⅱ Ⅲ aVR aVL aVF 預(yù)激綜合征 Preexcitation syndrome, Wolff ParkinsonWhite syndrome, WPW syndrome B型：“ Δ”向量對向左后， V1導(dǎo)聯(lián)“ Δ”波向下， R波消失或減少， S波加深； V5 、 V6導(dǎo)聯(lián)“ Δ”波向上， QRS波以 R波為主。 V1 V2 V6 Ⅰ Ⅱ Ⅲ 預(yù)激綜合征 Preexcitation syndrome, Wolff ParkinsonWhite syndrome, WPW syndrome C型：“ Δ”向量對向右前， V V2導(dǎo)聯(lián)的“ Δ”波及主波均向上， V V6導(dǎo)聯(lián)相反。 預(yù)激綜合征多見于健康人，除少數(shù)發(fā)生頑固的室上速之外，一般預(yù)后良好。但預(yù)激綜合征的圖形改變，與束支傳導(dǎo)阻滯和心肌梗死相似，兩者可相互混淆，應(yīng)予重視。 （五）心肌缺血、損傷和梗死 myocardial ischemia, injury and infarction 絕大多數(shù)心肌缺血、損傷和梗死系由冠狀動脈粥樣硬化所引起，除臨床表現(xiàn)外，心電圖的特征性改變及其演變規(guī)律是確定診斷和估計病情的主要依據(jù)。 心肌缺血、損傷和梗死發(fā)生后，隨著時間的推移在心電圖上可先后出現(xiàn)相應(yīng)的心電圖改變。當(dāng)一個區(qū)域的心肌發(fā)生缺血時，從中心到其邊緣區(qū)域缺血的程度是不同的，也可在不同部位同時出現(xiàn)上述三種圖形改變。 （五）心肌缺血、損傷和梗死 myocardial ischemia, injury and infarction 冠狀循環(huán) coronary circulation 正常與異常冠狀動脈造影 normal and abnormal coronary artery angiography 心肌缺血 myocardial ischemia 心肌損傷 myocardial injury 心肌梗死 myocardial infarction 冠狀循環(huán) 竇房結(jié)支 右冠狀動脈 右室前支 右房支 右緣支 左房支 左旋動脈 左冠狀動脈前降支 左緣支 左室間隔支 左室前支 （前面觀） coronary circulation SA node B.
