【正文】 滿意，對(duì)降低母兒并發(fā)癥，提高預(yù)后有顯著意義。本資料中B組較A組、C組妊娠期高血壓疾病發(fā)生率稍高，其中A組妊娠期高血壓疾病6例，輕度子癇前期6例，B組妊娠期高血壓疾病9例，輕度子癇前期7例，但差異無統(tǒng)計(jì)學(xué)意義（P＞），可能與納入本研究的患者均為輕型GDM有關(guān)或樣本量較少有關(guān)。 關(guān)于妊娠期糖尿病診斷標(biāo)準(zhǔn)的探討新標(biāo)準(zhǔn)較舊標(biāo)準(zhǔn)診斷的妊娠期糖尿病發(fā)患率增加23倍[7]，增加的病患多為輕度妊娠期糖尿病患者，導(dǎo)致大量的醫(yī)療資源被卷入。因此認(rèn)為把注意力集中在輕型GDM患者是放錯(cuò)了位置，建議對(duì)新標(biāo)準(zhǔn)的實(shí)行應(yīng)進(jìn)行風(fēng)險(xiǎn)、成本、收益的評(píng)估。有學(xué)者提出，糖化血紅蛋白含量以客觀地反映取血前4 ～ 8 周血糖平均情況，與血糖的暫時(shí)性波動(dòng)、抽血時(shí)間及抽血前代謝波動(dòng)等干擾因素?zé)o關(guān)，近年來，國際上逐漸達(dá)成推薦使用HbA1c 對(duì)GDM 進(jìn)行診斷和檢測［9］，但需進(jìn)一步確定其診斷切點(diǎn)值。本資料表明, 2010年IADPSG標(biāo)準(zhǔn)較為嚴(yán)格, 會(huì)使納入人群增加，降低漏診率，但可以明顯降低母、兒合并癥的風(fēng)險(xiǎn)。因?yàn)樘谴x對(duì)母體的影響是逐漸的和連續(xù)的, 故不應(yīng)期待一個(gè)唯一的血糖閾值就能夠把正常和異常糖代謝的孕婦完全分開。參考文獻(xiàn)[1] Bellamy L，Casas JP，Hingorani AD，et 2 diabetes mellitus after gestational diabetes:a systematic reviewand meta－analysis[ J] .Lancet，2009，373:1773－1779.[2 ]International Association of Diabetes and Pregenancy StudyGroups Consensus Panel，Metzger BE，Gabbe SG， association of diabetes and pregancystudy groups remend ations on the diagnosis and classification of hyperglycemia in pregancy[ J] .Diabetes Care，2010，33:676－682.[3 ][M].第7 : .[4] Bellamy L,Casas JP, Hingorani A D, et al.
