Reportable Injuries, Diseases and Dangerous Occurrences 應報告的傷害,疾病和危險事故 Definition 定義 ? When someone dies or suffers a major injury in a accident connected with the business (see definition of major injury below) 與企業(yè)有關的事故導致人員 死亡或受重傷 (參照以下主要傷害的定義) ? An employee is injured which results in immediate hospital admission for more than 24 hours 員工受傷而導致 住院 24 小時以上 ? There is a dangerous occurrence (see definition below) 危險事件 發(fā)生 (參照以下定義) Definition of Reportable Major Injuries 應報告的重要傷害定義 ? Work related fatality 因工作導致的致命傷害 ? Fracture other than to fingers, thumbs or toes 手指,拇指和腳趾以外的骨折 ? Amputation 切斷手術 ? Dislocation of shoulder, hip, knee or spine 肩膀、臀部,膝蓋或脊椎脫臼 ? Loss of sight (temporary or permanent) 失明(暫時或永久的) ? Chemical or hot metal burn to the eye or any perating injury to the eye 眼部受到化學物或熱金屬的燒傷,或是眼部受到的任何尖銳的傷害 ? Injury resulting from an electric shock or electrical burn leading to unconsciousness or requiring resuscitation or admittance to hospital for more than 24 hours 因電擊或是電燒傷而導致昏迷的傷害或任何需要住院 24 小時以上的傷害。 Unconsciousness caused by asphyxia or exposure to harmful substance or biological agent 因暈厥導致的不省人事,或是受到有害物體或生物試劑侵害而導致意識不清 ? Acute illness requiring medical treatment, or loss of consciousness arising from absorption of any substance by inhalation, ingestion or through the skin 任何需要醫(yī)療的急性疾病,或是因通過吸入,攝入或皮膚滲透物品而導致的意識喪失 ? Acute illness requiring medical treatment where there is reason to believe that this resulted from exposure to a biological agent or its toxins or infected material 需要醫(yī)療的急性疾病,并有理由相信該疾病是受到生物試劑或受其毒性或感染物品的侵害而導致的。 Over Three Day Injury 三天以上傷害 Over 3 day injuries to staff must be reported when a member of staff is incapacitated for work for more than three consecutive days, not counting the day of the accident but including any days which would not have been working days. The expression ‘more than three consecutive days’ means, in practice, at least four days. For example, if a person who normally works Monday to Friday is injured on Friday and returns to work the following Wednesday, the Saturday and Sunday would have to be included when counting the days of incapacity. The total period of incapacity would be four days and the injury must be reported. 如有 員工無法工作 連續(xù)三天 (不含 )以上 ,不包括事故發(fā)生的當天,但包括任何非工作日,就應該將該員工三天以上傷害這一事件進行報告?!斑B續(xù)三天以上”這個概念事實上就指的是至少有四天。比如,一位周一至周五正常工作的員工,于周五受傷,并于下個周三返回工作,當計算無法工作的天數(shù)時候應將周六和周日包含在內。這個無法工作的期間有四天,則這一傷害事件必須給于報告。 Definition of Reportable Dangerous Occurrences “ 應報告的危險事件”的定義 ? Collapse, overturning or failure of loadbearing parts of lifts and lifting equipment 任何載重的升降機或升降設備崩潰,倒塌或發(fā)生故障 ? Explosion, collapse or bursting of any closed vessel or associated pipework 任何封閉的容器或相關的管道發(fā)生的爆炸,崩潰或爆裂 ? Plant or equipment ing into contact with overhead power lines 任何設備與天花板電線發(fā)生碰撞 ? Electrical short circuit or overload causing fire or explosion 因短路或超負荷工作而導致的火災或爆炸 ? Any unintentional explosion, misfire, failure of demolition to cause the intended collapse, projection of material beyond a site boundary, injury caused by an explosion 任何非故意的爆炸,熄火或引爆失敗導致場所外圍的坍塌或物體射出,從而引爆炸引起傷害 ? Accidental release of a biological agent likely to cause severe human illness 生物藥劑的可能會導致嚴重疾病的意外外泄 ? Malfunction of breathing apparatus while in use or during testing immediately before use 使用中或是在測試階段即 將使用的呼吸設備發(fā)生故障 ? Traumatic inflammation of the tendons of the hand or forearm or of the associated tendon sheaths. 因重復活動造成的手或前臂抽筋。手或前臂腱關節(jié)或是相關的腱鞘外部發(fā)炎。 ? Some skin diseases such as occupational dermatitis. 一些皮膚疾病,比如職業(yè)性皮炎 ? Lung diseases including。 occupational asthma, pneumoconiosis, asbestosis, mesotherlioma。 肺部疾病包括:職業(yè)性哮喘,肺炎,石棉沉滯癥和間皮瘤。 ? Other conditions such as: occupational cancer, certain musculoskeletal disorders and hand arm vibration sysndrome 其他情況比如 : 職業(yè)的癌癥,肌骨胳類紊亂病和手臂振動癥。 Diseases are only reportable if diagnosed in writing by a Doctor, and the persons job involves the appropriate work activity.
