For further information, please call Insurance Service Hotline on (852) 2867 8678.「摯關(guān)懷醫(yī)療計(jì)劃」「摯關(guān)懷醫(yī)療計(jì)劃」條款及細(xì)則 1.投保資格下列申請(qǐng)人的家屬，有資格受保此計(jì)劃： 75歲以下的合法配偶 (若投保公司增值計(jì)劃或門診保障則為65歲以下) 所有未婚并受供養(yǎng)的合法子女，包括繼子女和合法領(lǐng)養(yǎng)的子女，唯年齡須介乎15天至17歲（包括首尾年齡）或23歲以下而在學(xué)校、學(xué)院或大學(xué)就讀的全日制學(xué)生。 2.保費(fèi)繳付方法保費(fèi)須直接在指定的信用卡戶口內(nèi)扣除。 3.續(xù)保安排 「續(xù)保通知書」會(huì)于您的「摯關(guān)懷醫(yī)療計(jì)劃」保單到期日之前45天內(nèi)寄往最后所知的通訊地址。隨后應(yīng)繳付的每年保費(fèi)會(huì)根據(jù)您投保時(shí)所指定的信用卡戶口內(nèi)扣除。當(dāng)收到續(xù)保保費(fèi)后，我們會(huì)發(fā)出新的醫(yī)療卡及保障項(xiàng)目表。 本保單將自動(dòng)及保證可終身續(xù)保。續(xù)保保費(fèi)及條款可能與到期之保單不同及會(huì)由安盛保險(xiǎn)有限公司全權(quán)厘定。 4.更改投保計(jì)劃在保單承保期內(nèi)，不能更改保障計(jì)劃。您可于續(xù)保時(shí)更改保障計(jì)劃，但必須于保單到期日前1個(gè)月遞交有關(guān)申請(qǐng)。有關(guān)更改將于批核后才開始生效。5.保費(fèi)退還申請(qǐng)人須預(yù)繳全年保費(fèi)，而有關(guān)保費(fèi)的退回安排，將依據(jù)保單退還保費(fèi)條款而定。除非申請(qǐng)人于接獲保單后15天內(nèi)要求取消該保單。6.私隱與保安聲明您在網(wǎng)上申請(qǐng)「摯關(guān)懷醫(yī)療計(jì)劃」時(shí)，將被視為已閱讀、明白及同意所列之「收集個(gè)人資料聲明」及「私隱與保安聲明」。倘若您尚未閱讀該聲明，請(qǐng)按「收集個(gè)人資料聲明」及「私隱與保安聲明」。 7.門診保障若您己揀選門診保障于網(wǎng)絡(luò)醫(yī)生求診時(shí)必需出示有效摯關(guān)懷醫(yī)療卡，否則有關(guān)門診保障將不獲賠償。有關(guān)條款及細(xì)則的詳情，請(qǐng)參閱保單。查詢?cè)斍?，?qǐng)致電保險(xiǎn)服務(wù)熱線(852) 2867 8678。Golfer39。s Insurance PackageDeclaration for Golfer39。s Insurance Package1.I (Insured)/We (Insured and insured39。s spouse) have read and understood the Personal Information Collection Statement.2.I/We understand that this application will form the basis of my/our contract with AXA General Insurance Hong Kong Limited.3.I/We hereby apply for Golfer39。s Insurance Package within the Hong Kong Special Administrative Region and declare that the statements made in or in connection with this application are true and plete and that I/we have not withheld any material facts which are likely to affect acceptance or assessment of the insurance cover requested. I/We understand that in case of doubt as to whether AXA General Insurance Hong Kong Limited should be informed of certain facts, I/we should call Insurance Service Hotline on (852) 2867 8678. Failure to disclose any material facts referred to above may mean that the Policy will not provide the Insured Person(s) with the required coverage, or may invalidate the Policy altogether.
