【正文】 Payment 出版日期 匯款日期 Date of Broadcasting Publishing Date 價格 總計 Rate Total Amount 備注 Remarks 本合同包括下列條款： 1．廣告樣稿 為合同附件，與本合同具有同等法律效力。 展團之外的刊戶，在送交稿件的同時， 必須將刊費一并交清，方予刊登 ，如刊戶中途撤稿，需酌情收取手續(xù)費。 五、 價格 黑白文字稿 每頁 1500 元 黑白圖文稿 每頁 2500 元 彩色內(nèi)插頁 每頁 6000 元 彩色扉頁 每頁 8000 元 封 面 每頁 40000 元 特殊版面 價格另議 封 底 每頁 30000 元 封 二 每頁 20200 元 封 三 每頁 20200 元 企業(yè)標(biāo)識 500 元 /個 六、 注意事項 刊登產(chǎn)品宣傳文稿的單位，需有企業(yè)所在省、自治區(qū)、直轄市醫(yī)藥管理部門審查批準(zhǔn)的 《醫(yī)療器械廣告審查表》及廣告審查批準(zhǔn)文號。文件中所用的特殊字體轉(zhuǎn)換為路徑或刻在光盤中與文件一起提供。 彩色圖文稿每頁圖片不超過 3 幅 (提供制版膠片者除外 ), 超出 3 幅的，每增加 1 幅加收 200元制版費。 為方便改動，提供的出片文件最好是用排版軟件做好，不要轉(zhuǎn)成圖象格式。 提供光盤或軟盤者 必須提供打樣 。 展團稿件一律發(fā)電子郵件。大會指定展位搭建商名單請參看《參展商手冊》相關(guān)附錄。新產(chǎn)品只允許進(jìn)行展示，禁止交易。 According to the ‘ Medical Equipment Production Supervising Regulation’ promulgated by the Chinese Central Government, all medical exhibits must be registered in State Food and Drug Administration (SFDA) before being displayed and sold in Chinese medical shows. All exhibitors participating in China International Medical Equipment Show (CMEF) should take the Registration Certificate of your medical equipment exhibits, and the officials from SFDA will do inspection around during the show opening days. Failure to submit registration information of your exhibits may result in the Authorities from SFDA ordering the closure and removal from the Exhibition hall of the exhibits or giving penalty to the exhibitor in the event, in that case the Organizer of CMEF will not be held liable for any loss thereby suffered by any exhibitor. The Organizer will set up the Exhibitor Database in reference to this Exhibitor Registration Form in order to make exhibit information searching much easier.
