婦科腹腔鏡手術(shù)中轉(zhuǎn)開(kāi)腹36例臨床分析(已修改)

【正文】 摘要：目的探討婦科腹腔鏡手術(shù)中轉(zhuǎn)開(kāi)腹的臨床因素。方法回顧性分析2000年1月～2007年12月 該院1 143例婦科腹腔鏡手術(shù)36例中轉(zhuǎn)開(kāi)腹患者的臨床資料。%。中轉(zhuǎn)開(kāi)腹的 原因依次為：盆腹腔粘連23例（%）、術(shù)中操作困難或并發(fā)癥8例（%）、惡性腫瘤3例（%）。有下腹 部手術(shù)史和開(kāi)展腹腔鏡初期中轉(zhuǎn)開(kāi)腹率較高。結(jié)論婦科腹腔鏡中轉(zhuǎn)開(kāi)腹的主要原因?yàn)榕韪骨徽尺B。術(shù)前根 據(jù)腹腔鏡技術(shù)操作水平，選擇合適的病例可以降低腹腔鏡手術(shù)中轉(zhuǎn)開(kāi)腹率. 關(guān)鍵詞：婦科腹腔鏡手術(shù)；中轉(zhuǎn)開(kāi)腹；盆腹腔粘連 Abstract:【Objective】To probe the reasons of conversion to laparotomy during gynecological laparoscopic surgery.【Methods】Clinical records of 36 cases of conversion to laparotomy out of 1143 cases of gynecological la－ paroscopic surgery from January 2000 to December 2007 were analyzed retrospectively.【Results】The rate of con－ version to laparotomy was %.The reasons of conversion to laparotomy were pelvic adhension in 23 cases,tech－ nical difficulty and plication in 8 cases,malignant tumor in 3 rate of conversion to laparotomy in pa－ tients with a history of laparotomy was significantly higher than that without a history of rate of con－ version to laparotomy in early stage of developing laparoscopy was significantly higher than
