【正文】 巖下竇 腦靜脈系統(tǒng)組成 腦靜脈系統(tǒng)組成 ● 腦靜脈 (cerebral veins) ◆ 淺靜脈組 大腦上靜脈、大腦中淺靜脈、大腦下靜脈 ◆ 深靜脈組 大腦中深靜脈、基底靜脈、大腦內(nèi)靜脈、 大腦大靜脈 (Galen vein)。 腦靜脈系 血栓形成 病因 ● 病因分類 ◆ 非感染性原因： 嚴重脫水、消耗性疾?。◥阂嘿|(zhì)、晚期癌癥）、某些血液病（溶血性貧血、鐮狀細胞病、血小板增多癥、抗凝血酶 III缺乏等）、先天性心臟病、充血性心力衰竭、妊娠、產(chǎn)后、口服避孕藥、創(chuàng)傷、手術(shù)后、自身免疫性疾病、高熱、休克、腎病綜合癥、酮癥酸中毒、陣發(fā)性睡眠性血紅蛋白尿、抗磷脂抗體綜合征、蛋白Ｓ和蛋白Ｃ缺乏癥等。 ◆ 慢性起?。? 病情進展超過 30天 ， 30%。 ◆ 以運動障礙和感覺障礙為主 ， 下肢為著 。 ● 具有可逆性 ， 但通常提示預(yù)后不佳 。 ● 此竇是腦皮層靜脈循環(huán)和腦脊液回流的必經(jīng)之路 。 范圍廣泛，超過上矢狀竇 2/3以上 位于上矢狀竇后方 局限于上矢狀竇前段 累及到額葉前部靜脈回流 波及到中央回靜脈 影響到其它大腦淺靜脈 ● 起病形式 腦靜脈 竇血栓 上矢狀竇血栓 （ superior sagittal sinus thrombosis， SSST） 腦靜脈 竇血栓 上矢狀竇血栓 （ superior sagittal sinus thrombosis， SSST） 腦靜脈 竇血栓 上矢狀竇血栓 （ superior sagittal sinus thrombosis， SSST） 腦靜脈 竇血栓 上矢狀竇血栓 （ superior sagittal sinus thrombosis， SSST） 腦靜脈 竇血栓 上矢狀竇血栓 （ superior sagittal sinus thrombosis， SSST） 腦靜脈 竇血栓 上矢狀竇血栓 （ superior sagittal sinus thrombosis， SSST） This 58 yearold man plained of a severe headache 12 hours after taking part in the London marathon. He attended the general casualty but no cause was found. He was discharged with a diagnosis of possible migraine. However, the pain persisted and he reattended the casualty 2 days later. He was found to have bilateral optic disc swellings and an urgent MRI scan was requested. Full blood count revealed high platelet count (thrombocythemia) but otherwise all the other tests were normal 腦靜脈 竇血栓 上矢狀竇血栓 （ superior sagittal sinus thrombosis， SSST） Clinical History: 17yearold female who presents with a four day history of headaches and a seizure immediately prior to admission. There is no significant past medical history. Findings: An axial noncontrasted CT image through the brain demonstrates cerebral edema within the left frontal lobe and deep gray matter. In addition, there is hyperattenuation within the region of the straight sinus. Petechial hemorrhage as well as edema is seen within the left frontal lobe An angiogram demonstrates occlusion of the anterior one half of the superior sagittal sinus as well as occlusion of the deep venous system bilaterally. 腦靜脈 竇血栓 上矢狀竇血栓 （ superior sagittal sinus thrombosis， SSST） ● 預(yù)后： 單純上矢狀竇血栓在數(shù)周或數(shù)月后可再通 ， 顱內(nèi)壓恢復(fù)正常 ， 預(yù)后良好 。 腦靜脈血栓形 成的臨床表現(xiàn) 大腦半球受損 ● 局灶性神經(jīng)功能缺損 ◆ 發(fā)病初期： 15%；病程中： 60%。 腦靜脈血栓形 成的臨床表現(xiàn) 大腦半球受損 ● 發(fā)病初期少見 ， 約有 4%； 發(fā)病中期：近一半患者有不同程度的醒覺水平下降 。 腦靜脈血栓形 成的臨床表現(xiàn) 全身癥狀 按部位 CVT可分為： ● 靜脈竇血栓 ● 大腦淺靜脈 (皮層靜脈 )血栓 ● 大腦深靜脈血栓 ● 小腦靜脈血栓 ● 頸靜脈血栓 各種腦靜脈血栓的臨床特點 ● 上矢狀竇血栓 ● 側(cè)竇 (橫竇 及乙狀竇 )血栓 ● 巖上竇及巖下竇血栓 ● 直竇血栓 ● 海綿竇血栓 腦靜脈竇血栓 ● 其解剖特點決定了血栓易在此部位形成。 腦靜脈 竇血栓 上矢狀竇血栓 （ superior sagittal sinus thrombosis， SSST） ● 血栓的部位 、 累及范圍和梗阻程度決定了損害情況 。 ● 病因 以耳或乳突的急慢性感染最為常見。 ◆ 當對側(cè)橫竇或竇 匯先天異常，顱內(nèi)靜脈僅依賴一側(cè)橫竇 回流 易導(dǎo)致顱內(nèi)壓增高。 ■ 血栓 經(jīng)竇匯或下吻合靜脈擴展到上矢狀竇 時，會產(chǎn)生嚴重的顱 內(nèi)壓增高、昏迷、癲癇、偏癱或截癱。 血栓波及此竇，可出現(xiàn)同側(cè)面部疼痛或感覺減退。 ◆ 單純直竇血栓少見，常合并其他部 位血栓。 蝶竇、篩竇通過篩靜脈或直接接觸感染，侵入蝶竇壁 而入海綿竇。 海綿竇血栓 (cave
