【正文】 我明白及同意以此日期生效及簽署 Declaration ＆ Signature 聲明及確認： 1. Member should fill in correct information. All information will be kept for internal use only. 會員需填寫正確資料。所有資料只供內部使用。 2. Membership will be effective immediately when full payment is made. 會籍將于繳清余款當日開啟。 3. All fees are nonrefundable under any circumstances. 任何情況下，所有預繳費用將不予 退還。 4. A replacement fee shall be charged for any loss of locker key and membership card. 會員如遺失會員證或儲物柜鑰匙，本會所將向會員征收補領之手續(xù)費。 5. Member must sign on the beauty/massage record for each session at the time of treatment. 會員需就私人教練服務簽署個人完成記錄。 6. All personal training sessions must be pleted on or before this contract expires. 所有私人教練服務均需全在本合約屆滿時或之前完成。 7. All personal training service will begin only after contract amount is paid in full. 會員需全繳支付合約款項方可使用私人教練服務。 8. All personal training sessions are not refundable and cannot be used to offset against any membership account balance. 私人教練服務均不可退回，亦不可用以抵消任何會員賬戶余額。 9. A 24hour cancellation notice is required for any personal training sessions otherwise the member will be charged for the nonattendance. 會員如要取消私人教練服務，必須于 24 小時前通知會所，否則需支付全部有關費用。 10. ZHEJI reserves all rights for the special offer in this application. 哲擊保留所有優(yōu)惠最終解釋權。 11.
